Avian Health Forms
The purpose of these forms is to help bird owners track down the cause or causes of their bird's ailment. This form is also used by members of the HolisticBird list who wish to ask other list members for help. By answering the questions, the bird owner is guided through a troubleshooting process that often helps target the solution. It is an aid to diagnosis, without which, treatments cannot be suggested.
This form is to be used for any of the following:
1. your own personal problem solving. Print out the form. (use your mouse to highlight the form and then click on file/print from the menu bar at the top of the page or whatever other method you use to print pages.)
2. to bring along on your visit to the vet to help with diagnosis. In this case, you would print out the form and then answer the questions
3. to send to the HolisticBird list, if you are a member of the list, and would like to ask the other list members for help. If you are not a member of the HolisticBird list, Yahoo groups will reject the mailing.
Please try to give as much information pertaining to the problem no matter how insignificant they may appear. The importance of some details might not be readily apparent to the owner, but may be important clues to others who are more experienced.

Everyone doesn't need to fill out every form. Which forms should you fill out?
1. If your bird is sitting on the bottom of the cage fluffed up with its eyes closed, or its tail is bobbing even though it isn't being disturbed, if it is bleeding or has consumed a toxic substance, if it has inhaled gas, teflon or smoke from a fire or burned food, then do not fill out the form. Your bird is in an emergency situation and must be taken to a veterinarian immediatly.
2. If your bird has a behavior problem such as screaming, biting or other aggression, it probably isn't necessary to fill out forms H, I, J, or K.
3. If your bird is plucking or mutiliating its feathers, fill out all of the forms.
4. If your bird has minor diarrhea or looks as if it isn't feeling well because it is quieter than usual and isn't eating as well, fill out forms A, C, D, E.
5. If your bird has been to the vet, fill out form B.

Normal droppings consist of three parts: 1. solid feces of different colors depending on what the bird is eating; 2. clear urine; and 3. white urates. Droppings might be watery, slimy with mucus, well formed tubes, dry / hard, black /scant, contain whole seeds or other food, foamy, the urates or urine could also be colored.etc Describe your bird's droppings including solids, urates, and urine. |
